AGES 12-15 ARE NOW APPROVED TO RECEIVE THE VACCINE!!! (MUST BE ACCOMPANIED BY A PARENT OR GUARDIAN)

Westside has been approved to be a COVID-19 Vaccination site!!! Please share this information with EVERYONE YOU KNOW!! We are looking to vaccinate 300+ people in our community. Westside in conjunction with Decatur Morgan HH will be administering the 2-shot Pfizer vaccine.

Forms can be picked up from the church OR you can also download them by clicking the download button below, print it, filled it out and return to the church by dropping it off or emailing it to us at wmbcdecatur@gmail.com.

Forms can be returned to the church each Sunday from 9am to 10am all the way up to the date at 909 4th Street NW Decatur AL

Walk in appointments will be accepted on the day of the clinic.

When: June 5th (2nd Shot and FIRST TIME SHOTS WILL BE GIVEN AS WELL)
